MEDICAL SKIN TREATMENTS

 

  • Chemical peel (glycolic acid) - Chemical skin peeling involves an application of a mild fruit acid (glycolic acid) to sun-damaged, unevenly pigmented, and finely wrinkled facial areas.  It also works well to de-oil the skin and help acne improve rapidly.   The procedure  painlessly removes the skin’s top layers, revealing the healthier skin below.

 

We use only Neostrata brand chemical peel solutions.  We will choose the strength that is best for you (20-70% is available).  After cleansing your face, we use a fine brush to apply the medicine to to the areas to the entire face.   There is a mild tingling feeling that lasts about 3-5 minutes.  We then apply a neutralizing solution and a pleasant facial mask.  The whole procedure takes less than 10 minutes!

You will leave the clinic with minimal redness which can be covered with some powder immediately.  Over the next few days you may see some dryness or light flaking of the skin. You may use makeup or just moisturize a little. Make sure you stay out of the sun.
For best results, repeat the peel in two or three weeks.  By doing a series, we target many layers of the skin and achieve a radiant, clear complexion!

 


 






MICRODERMABRASION


What is microdermabrasion?

Microdermabrasion is a new way to rejuvenate and revitalize your skin in less than 30 minutes. The procedure uses a diamond wand with a powerful vaccuum to smooth and polish the skin. We remove the dead outer layer of skin, resulting in new, softer skin with a more even texture. Because microdermabrasion is so precise, the procedure is pleasant and  pain-free.

 

What can microdermabrasion treat?

Microdermabrasion is ideal for treating sun-damaged skin,   fine lines, wrinkles, oily pores, blackheads, whiteheads, and acne scars. The neck and chest and back can also be treated to blend the skin and remove any sun damage often found in that area.

Best results are seen when treatments are repeated every 2-3 weeks.   This is an excellent procedure to use as maintenance for a glowing complexion!

 

How long does the treatment take? How many treatments will I need?

One of the main benefits of microdermabrasion is that the entire procedure can be done in as little as half an hour. A complete microdermabrasion program is usually a series of treatments depending on your specific needs.

ACNE


Acne is a skin condition which has plugged pores, inflamed pimples,  and deeper lumps and cysts.   Although most teenagers get some form of acne, adults can also develop acne.  To avoid permanent scarring, treating acne is important.

 

Treatment:

Control of acne is an ongoing process over at least  two to three months,  depending on your type of acne.   We may prescribe medication for you, or suggest treatments that will help clear your skin as quickly as possible.The medicines and treatments we have available are listed here:

 

Topicals

  • we may prescribe topical creams, gels, or lotions with vitamin A acid-like drugs, benzoyl peroxide, or antibiotics to help unblock the pores and reduce bacteria. These products may cause some drying and peeling.   
  • Microdermabrasion may be used to remove the upper layers of the skin improving irregularities in the surface, contour, and generating new skin.
  • Light chemical peels with  glycolic acid help to unblock the pores, open the blackheads and whiteheads, and stimulate new skin growth.
  • Injections of corticosteroids may be used for treating large red bumps, painful cysts or pimples. This may help them go away quickly.
  • Laser treatments such as IPL and Medlite Q-switch NdYag are available.
  • Photodynamic therapy using the blue wavelength of light can be helpful in treating acne as well.  The Blu-U light treatment is available on Guam.  Come twice a week for a 16 minute treatment, approximately 12 times.  We can intensify the effect of the treatment by using a topical medication first.  It will take about 30 minutes to take effect, prior to applying the blue light.

 

Oral medication to be taken by mouth

  • Antibiotics taken by mouth such as tetracycline, doxycycline, minocycline, or erythromycin are often prescribed.
  • Birth control pills may significantly improve acne, and may be used specifically for the treatment of acne.
  • In cases of unresponsive or severe acne, isotretinoin (Accutane) may be used.  Pregnancy must be prevented while taking the medication, since the drug causes birth defects.

  • Scar treatment - Acne scars are textural abnormalities of the skin that are left behind after pimples heal. These range from shallow depressions, to sharp "ice pick" pits, to elevated bumps. Pimples can also heal leaving brown or red discoloration behind, but these generally fade over time without treatment and are not considered true scars.
  •  

    A topical bleaching cream can be used to speed up the fading of the brown discoloration. It is important to wear sunscreen whenever going outside to keep the scars from turning darker.

     

    What are the treatment options for acne scars?

    Mild scars can also be improved with microdermabrasion. This technique uses vacuum and a diamond-tipped hand piece to buff the superficial skin.
    Soft depressed scars can be filled in with filler material such as Restylane.
    We also offer Intense Pulse Light (IPL) and Nd:Yag Laser treatment, which are helpful in skin rejuvenation and improvement of acne scars.

     

    How many treatments are necessary?

    Microdermabrasion requires multiple treatments, initially at two- to three-week intervals. IPL requires 4-5 treatments, about 3 weeks apart.


    How long will it take to heal?
    Recovery time is minimal; most people can get right back to work after a treatment session.

     

    What if my scars are deep?

    One of the most powerful treatments for deep acne scars is medical needling. After numbing your skin, we gently move a roller full of tiny needles across your skin. This breaks up the scars and allows for new healing as much smoother skin. Effects are seen within a few weeks and the procedure is usually repeated every 6-8 weeks until you are satisfied. 

     

    Cost is $300 per session.  Downtime is one day.

How do lasers remove unwanted pigments in the skin?

Laser therapy is one of the most exciting treatments for removing unwanted skin pigments such as birthmarks, age spots, freckles and sunspots. Lasers work by emitting a specialized light that passes through the skin and is preferentially absorbed by the unwanted pigment ( natural melanin) in the skin. The laser causes the pigments to break up and disappear, leaving you with smooth, clear skin.

 

What can I expect from the procedure?

We numb your skin first with a topical anesthetic cream. Most people describe the treatment as feeling like the light “ snap” of  a rubber band against the skin. Any discomfort can be eased with the application of cool compresses. After treatment, you can immediately return to your normal activities. You may choose to wear make-up to cover the mild redness that follows the procedure.

 

Is it permanent?

After a treatment, you will notice the dark spots “flaking off” or lightening over a period of several weeks. It is important to stay out of the sun and use a good sunscreen during this time. Best results are usually obtained with several treatments, spaced about three weeks apart.

WRINKLE REMOVAL

 

What causes lines and wrinkles?

Wrinkles are caused by changes in the skin associated with aging and sun exposure. In some cases, they are also caused by the contraction of underlying muscles. This is especially true around the eyes and mouth. Smoking aggravates wrinkle formation.


What are the treatment options for wrinkles?

There are numerous treatments for lines and wrinkles. Topical tretinoin (RetinA) and alphahydroxy acids ( ASAP) are beneficial.  Glycolic acid peels and microdermabrasion work by stripping away the top layers of the skin. Lines can be filled in by injecting a filler substance such as Restylane.,
The action of muscles causing certain lines can b e weakened by using botulinum toxin, commonly called Botox.
Skin rejuvenation can be accomplished with Intense Pulse Light (IPL), which reduces the fine lines that come with aging.

Choosing the best approach depends upon an individual's lifestyle and goals.


Prevention

Wrinkles are best prevented by minimizing sun exposure and using protective clothing and hats. Always use a good sunscreen with at least SPF 30 on Guam.  A good skin care regimen with moisturizers is helpful. A vitamin/anti-oxidant supplement may be beneficial.

 

  • Botox® (20 units per area) - Botox® Cosmetic is a purified protein that is injected at very low doses into the face to relax deep, persistent lines produced with facial expressions. Botox® works by reducing the contractions of the muscles that cause wrinkling. The effects of treatment can last three months or longer in some cases. Results have been reported to last longer with repeated treatment.

We inject Botox ® to treat moderate to severe glabellar, or frown lines, crow’s feet, which are lines around the eyes, lines around the mouth and forehead.


Botox may be injected into the corner of the jaw to create a thinner, more oval face.  You may need to avoid chewing hard foods for several days after the procedure.  The effects occur gradually over about a month.   Contouring lasts 6 months to a year. Cost is $550.

 

 

  • Restylane filler - We use cosmetic fillers to rejuvenate facial skin by smoothing out everything from small wrinkles caused by smiling, wrinkles between the eyebrows, as well as correcting deep furrows and sagging cheekbones.  Fillers are also used to correct hollowness around the eyes, and add fullness to thin lips. If your face is disproportionate, that can be improved, too.

Fillers can be used to restore fullness to the face that has lost fullness with age, creating more prominent cheekbones or a more angled jawline.

Fillers are also used to enhance the appearance of upper and lower lips. We have also used fillers to treat acne, chicken pox, and other deep scars.

What do the Fillers Contain?

We use the filler Restylane® .  Restylane® is made from hyaluronic acid, a substance that naturally creates volume when injected into the skin It is completely natural and involves no foreign substances. Restylane® lasts approximately four to six months.

What is laser hair removal? How does it work?

Laser hair removal offers patients a safe, fast, and effective alternative for dealing with unwanted hair. Lasers work by emitting a specialized light that is passed through the skin and preferentially absorbed by its target, in this case, hair. The laser used in hair removal targets the melanin (color) in the hair follicle, damaging it and removing the hair. Laser hair removal is safe enough to be used on all body parts, including the face, back, underarms, bikini line, legs, and arms.

 

Is treatment permanent?

In most cases, about 80% permanent hair removal can be achieved after several treatments. Remember that the goal may not be total hair removal. Many patients are happy with a reasonable degree of hair reduction.
Also remember that the hair that remains is often lighter in color and thinner than the original hair prior to laser surgery. As with other treatments for unwanted hair, there is no guarantee that laser hair removal treatment will be permanent.

 

How many treatments are necessary?

Factors such as the amount of hair and individual reaction to the laser treatment will affect the number of treatments needed. In most cases, three to six treatments will be necessary to achieve optimal results, because there are always some hairs in a “resting phase.”

 

Does treatment hurt?

What can I expect with the procedure? We numb your skin with ice before treatments. Most patients describe laser hair removal as feeling "prickly,”. Afterwards, it might feel like a sunburn. Each patient has his or her own level of skin sensitivity and pain threshold. Different areas on the body are also more sensitive than others. Normally the skin just looks red for an hour or two. LASER TATTOO REMOVAL

Tattoo Removal

In the past, if you had a tattoo, you had it for life. Now, you can get rid of it in several sessions, a few weeks apart!
A laser produces a beam of highly concentrated light. Different types of lasers produce different colors of light.  The color of light produced by a particular laser is the key to the laser's effect on tattoo ink.
Particular colors of light are absorbed by specific colors of tattoo ink; for example, green ink is absorbed by red light.  The laser's light energy vaporizes or fragments the targeted ink.  The body then absorbs the tiny particles of ink, and the color fades.
Removing a small tattoo of one color may take two or three sessions, while multicolored tattoos may take several sessions, depending on their size and type of ink.  Sessions are usually spaced 4 - 8 weeks apart and are performed at EXPRESSCARE!

Does it Hurt?

The laser treatment itself is minimally painful.  Patients say it is less painful than when the tattoo was first created.  It feels like snaps with a rubber band.  We can use ice or numbing cream or even an injection of anesthesia  for those who are more sensitive to pain.

What Can I expect after treatment?

Immediately after treatment, there is redness, light crusting, or tenderness, all of which disappear in a short time.  The area may scab up for a few days.  No special care is required, except for keeping the area clean and staying out of the ocean until it has healed.

KELOID SCAR TREATMENT

How can scars be improved?

Scars that are raised or thickened (keloids) can be softened or flattened with intra-lesional cortisone injections. The skin is numbed first, with a topical anesthetic cream and then a tiny needle. The keloid shrinks gradually over several weeks. Several sessions are needed, about one month apart.


Other treatments include liquid nitrogen and silicon sheeting occlusion. The Nd:Yag laser can also be used to decrease the redness in a scar, and to shrink a keloid. Bleaching creams have been used to decrease brown discoloration in a scar with some success.

Is the treatment painful?

There is some discomfort with treatment. We try to make the numbing as effective as possible.

How many treatments are necessary? Are they covered by medical insurance?

All of these methods generally require multiple treatments, each about a month apart. They are not covered by medical insurance.

LIPO-DISSOLVE

 

What is Lipo-Dissolve?

For the perfectionist and body image-conscious person wanting to  get rid of stubborn fat pockets, Lipodissolve is the way to go. THIS IS NOT A WEIGHT-LOSS SOLUTION, but will get rid of small areas that just DON'T RESPOND TO EXERCISE. 


Lipo-Dissolve involves a series of injections of a medicine called phosphatidylcholine into problem areas to dissolve fat and take off inches.


The active ingredient phosphatidylcholine, is a naturally occurring substance in the body first used in the 50’s to reduce cholesterol and triglyceride levels in the blood or arteries. In the past decade however, other uses for this drug have become the latest craze in the field of cosmetic dermatology. Pioneered by Dr. Patricia Rittes of Brazil, she began using Lipostabil to reduce the fat pads that caused eye bags but has since used them on other fatty parts of the body.  Dr. Lom has trained with Dr. Rittes.


Although not a replacement for liposuction because it only dissolves small amounts of fat, it is perfect for the person who wants to lose the small "pinch-fat" on the tummy, thighs, "butt bananas" or armpit chub that hangs out of tank tops. 

How many sessions are required?

At least four sessions are required to see the full results of Lipo-Dissolve. These sessions are spaced two to four weeks apart.

What should I expect?

After each session it is normal to experience some bruising, itching and redness which will last anywhere from a couple of hours to a couple of days. Swelling and tenderness are to be expected for the next 1-2 weeks following each session. Some lumpiness or hardness may also occur in the treated areas but will smoothen out in the weeks to follow. Massage helps.  Some soreness and tightening will result, just as if you did some heavy work exercise.

What is the procedure like?

First, measurements of problem areas to be treated are taken. Ice is applied to cool and numb the skin.
After cleansing, phosphatidylcholine  is injected into the target areas.  A tight wrap is applied to reduce swelling.  Spandex clothing or a girdle really help with swelling and discomfort.
Lastly, an ice pack is applied and the patient is free to go when they feel ready.
A painkiller may be taken to alleviate any discomfort or pain. The treatment is repeated after 2 weeks.

How long is the downtime or recovery period?

There is no downtime. The patient is free to go out and do anything he or she likes. Some people are even known to go to the mall for dinner or a movie.

MOLE REMOVAL
Moles are smooth skin growths that may be flat or elevated and red-, brown-, or skin-colored. They may be present at birth or early childhood (congenital moles) or may develop in early adulthood (acquired moles).

What treatments are considered medically necessary vs. cosmetic?

Moles must be carefully evaluated by a qualified physician to ascertain any concern for skin cancer. Worrisome moles must be biopsied to rule out melanoma. These biopsies are considered medically necessary and are thus covered by medical insurance. Cosmetic removal of moles is not covered by insurance.
Skin tags are considered cosmetic, and are not covered by most insurance.

How are moles removed?

Raised moles can be removed surgically, either by excising the mole or by shaving it flush with the plane of the surrounding skin. If the mole goes very deep below the surface of the skin, there is always a chance that it may “come back” later. Flat brown moles can also be excised or treated with a number of lasers to decrease or remove the brown pigmentation of the mole. This last method generally requires more than one treatment.
Skin tags may be snipped, scraped, frozen or burned off. None of these methods is painful if a topical anesthetic cream is applied first.
